Health Forms

ALL students enrolled at Misericordia University, regardless of their housing status (resident or commuter) and/or their academic status (full-time or part-time), must submit a completed health form to the Health & Wellness Center prior to arrival on campus.  Failure to comply with this part of the admission process could affect your housing status.

Please be sure to make additional copies for your own records as you will be required to submit them to various departments especially if you plan on being involved with service groups, clinical rotations and/or athletics.
